Criminal Background Check and Mandated Reporter Forms

Please click on the link above for the needed forms to volunteer or chaperone at our school.  These forms are for individuals who plan to volunteer or chaperone for more than one opportunity or individuals who will work closely with our students.  You will need to complete all three forms in its ENTIRETY for proper processing.  You may return the forms to Mrs. Reynolds by fax at 770-474-5528, leave with the front office staff, or return with your child. Background Checks may take up to three weeks for processing.
