These are the necessary documents needed for registration.

• Original Birth Certificate for child enrolling (no copies)
• Parent or Guardian's Photo Identification (no copies)
• Proof of Guardianship
• Proof of Residency (current lease or current property tax statement)
• Current gas or electronic bill (no copies)
• Ear, Eye and Dental Records (on Georgia Form 3300)
• Immunization Record (on Georgia Form 3231)
• Social Security Card for child enrolling (no copies),
• or a signed social security waiver
